Overview

What is Semaglutide?
Semaglutide is a glucagon-like peptide (GLP-1) receptor agonist. It is used as an adjunct to physical activity and reduced calorie diet for obese (BMI ≥30) and overweight (BMI ≥27) patients with at least one comorbid condition.

How does it work?
Semaglutide works by stimulating glucose-dependent insulin release from the pancreatic islets, slowing gastric emptying, inhibiting inappropriate post-meal glucagon release, and reducing food intake.

Semaglutide/Hydroxocobalamin comes in 1mg/1ml and 2mg/ml concentrations.

Storage

Storage and Dosing
When stored in the refrigerator, each vial should be used up to 30-day period from the time of first puncture. Opened and unopened vials must be stored in the refrigerator to maintain efficacy. At room temperature, vials can be used for up to 24 hours.

Adjust dosage using the following schedule: In patients who do not tolerate a dosage increase, consider delaying the increase for an additional 4 weeks.

Weeks 1-4:
Semaglutide-Hydroxocobalamin 1 mg-1 mg/1 ml
Inject 0.25 ml (0.25mg) subcutaneously every week, dispense in 1 mL MDV for a one-month supply

Weeks 5-8:
Semaglutide-Hydroxocobalamin 1 mg-1 mg/1 ml
Inject 0.5 ml (0.5mg) subcutaneously every week, dispensed In 1 mL x 2 MDVs for a one-month supply

Weeks 9-12:
Semaglutide-Hydroxocobalamin 1 mg-1 mg/1 ml
Inject 1.0 ml (1mg) subcutaneously every week, dispensed in 4 mL MDV for a one-month supply

Weeks 13-16:
Semaglutide-Hydroxocobalamin 2mg-2mg/1ml
Inject 0.85 ml (1.7mg) subcutaneously every week, dispensed in 4 ml MDV for a one-month supply

Weeks 17+:
Semaglutide-Hydroxocobalamin 2mg-2mg/1ml
Inject 1.2 ml (2.4mg) subcutaneously every week dispensed In 5 ml MDV for a one-month supply

Suggested Dosage:
See above for recommended schedule. Galleria Medical Pharmacy does not recommend increasing dose above 1 mg of Semalgutide for moderate weight loss goals. However, it is up to the healthcare provider to determine the maximum dose per administration and duration of therapy.

Clinical Uses

Diabetes mellitus, type 2, treatment:
As an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.  This can cause risk reduction of major cardiovascular events (cardiovascular death, nonfat myocardial infarction, nonfatal stroke) in adults with type 2 diabetes mellitus and established cardiovascular disease.

Weight management, chronic:
As an adjunct to a reduced-calorie diet and increased physical activity for chronic weight management in adults with an initial BMI of ≥30kg/m2 (obesity), or 27kg/m2 (overweight) in the presence of at least one weight-related comorbid condition (e.g., hypertension, type 2 diabetes mellitus, dyslipidemia).
